Integra Balanced Medicaid Advantage (MA) (HMO SNP)
Integra Balanced MA (HMO SNP) combines your Medicare and most of your Medicaid benefits under one benefit package. Medicaid benefits excluded from our plan can be accessed by using your Medicaid fee for service card.

Eligibility
You may enroll in Integra Balanced MA (HMO SNP), if you meet the following criteria:

Have both Medicare Parts A and B

Have full Medicaid benefits (QMB+ and SLMB+)
-
Live in our service area of Bronx, Kings, New York, Queens or Nassau counties in New York

Be 18 years of age or older
Plan Information
With Integra Balanced MA (HMO SNP), you get all the benefits you would get under Original Medicare, plus more. In addition, Integra Balanced MA (HMO SNP) also your Medicaid benefits (some are excluded from our plan but are available by using your Medicaid card). With our plan, you pay $0 for all your covered Medicare and Medicaid benefits. Since you also qualify for “Extra Help,” you also get low cost prescription drug coverage.
